Cooling Off Period:

Once an invoice is issued and a deposit or balance is paid in full, you have a 14-day window after booking during which clients can cancel surgery and receive a full refund.

Cancellation After Cooling Off Period:

If cancellation occurs after the 14-day cooling off period, a $3000 cancellation fee applies. We suggest during this period you undergo the recommended surgery testing to ensure you are fit for surgery.

Cancellation within 29 Days of Surgery:

If surgery is cancelled less than 30 days before the scheduled date, the full payment for the surgery is forfeited. However, if the Azurite Package is rescheduled, the cost of surgery can be retained for a future date within 12 months. Outside of this period, additional charges may apply due to price fluctuations or currency changes.

Cancellation on the Day of Surgery:

If surgery is cancelled on the scheduled day, the cost of the surgery is forfeited. However, rescheduling is permitted in cases of medical necessity, with an additional AUD 2000 hospital fee incurred for cancellations made on the day of the surgery.

Flight disruptions during package dates

Our Azurite Surgery packages feature a two-day travel period preceding the surgery to accommodate potential flight-related issues. In the event of rescheduling due to flight disruptions during this period, we will make every effort to arrange a new surgery date within the originally planned package dates. Please note, however, that this accommodation is subject to availability and cannot be guaranteed.

Additional Fees for Cancelling During Stay:

If a client opts not to proceed with a scheduled procedure that has been paid for during their stay, they'll incur an additional accommodation package fee if they choose to have the surgery at a later date.

No Refunds for Certain Procedures on Promotional Packages:

Refunds are not applicable for surgeries not undergone by clients included in promotional or heavily discounted packages, except for surgery add-ons quoted separately from the package.

Extension of Package Days:

All packages include a standard number of hospital and accommodation nights. If a client needs to extend their package, Azurite will invoice them at a daily rate of AUD 150 for any additional nights – this rate is consistent throughout the year.

Aftercare with staff Post-Surgery Rehabilitation and Care

  1. Hospital Stay: Patients will remain in the hospital until medically deemed safe for discharge, typically ranging from 1 to 3 nights.
  2. Hotel Accommodation:
    • Accommodation at our affiliated hotel is provided where nursing assistance may not be available 24 hours a day.
    • Additional nursing assistance can be arranged for a fee of 125 AUD per day, subject to availability.

Assistive Devices: Upon discharge from the hospital and during your stay at the hotel, walking assistance devices or wheelchairs will be provided as needed for your mobility.

Superannuation Services:

We are committed to facilitating the compassionate release of superannuation funds for medically necessary surgeries. Upon approval, full payment is required. In the event of non-proceeding, a fee of 2000 AUD will be charged, unless there is evidence of application denial.

Additional hospital nights

Please be aware that we allocate standard timeframes for hospital stay for each surgery. Once you return to the hotel, you should be mobile and capable of self-care.

If you do not have a companion and require additional assistance, we offer full-time in-room care for an additional fee. Please contact us for more information. Additionally, any pre-existing medical conditions requiring extra medication or extended stay are the responsibility of the client, therefore we highly undertaking an insurance policy.

Azurite Medical and Wellness: Satisfaction and Resolution Policy

At Azurite Medical and Wellness, we are dedicated to ensuring our customers are satisfied with their surgeries. If any issues arise during your stay, please contact our team in Australia immediately for a prompt resolution. Any surgical warranty issues that occur during your stay in Bangkok will be rectified at no additional cost.

If you are dissatisfied once you have returned to your home country and are outside of our package coverage, a revision fee will of 3200 USD will apply.

Azurite is proud of its reputation as having one of the highest customer satisfaction rates in the industry and will make every effort to ensure your successful surgery and recovery. It is intended that these Terms and Conditions are legally binding on you and us. By completing the Client Registration Form and proceeding with payment (full and in part), you are deemed to accepting these Terms and Conditions, which together with the Privacy Statement govern Azurite’s relationship with you in relation to your surgery package

Azurite may change the content of the Surgery Guide, Terms and Conditions and Privacy Statement at our discretion and without notice. By continuing to proceed with the surgery quoted (or billed) you accept the Terms and Conditions as they apply from time to time. Your engagement with Azurite Medical and Wellness and any dispute arising out of the engagement is subject to the laws of New South Wales, Australia.

Azurite Medical & Wellness shall not be held responsible for any disputes arising from transportation, onsite team, or Azurite's staff. Any issues related to transportation, onsite team, or Azurite's staff shall be the sole responsibility of the client, and Azurite Medical & Wellness shall not be liable for any damages or losses arising from such disputes. The client agrees to indemnify and hold harmless Azurite Medical & Wellness, its officers, directors, employees, agents, and affiliates from any claims, damages, or losses arising out of or related to transportation, onsite team, or Azurite's staff. By booking our services, the client acknowledges and agrees to this clause.

  1. Pre-Surgery Health Screening:
    • A comprehensive health screening is mandatory for all patients prior to surgery. This includes, but is not limited to, blood tests, imaging studies, and a physical examination.
    • An HIV test is a required component of the pre-surgery health screening.
  2. HIV Policy:
    • If a patient's HIV test returns a positive result, we will be unable to proceed with the surgery. In such cases, the surgery will be canceled and all surgery fees will be fully refunded.
  3. Abnormal or Unhealthy Test Results:
    • If any tests indicate abnormal or unhealthy conditions that pose a risk for surgery, the surgery will be canceled.
    • In the event of a cancellation due to abnormal or unhealthy test results, all surgery fees will be fully refunded unless on day of surgery.

Quality of the final result of surgery is also dependent on the customer/ client closely following any health and healing recommendations given by Azurite or their affiliated surgeons. In the unlikely event that the outcome of the surgery is less than should be expected and is attributable to the surgical error, Azurite will pursue remediation of the surgery at no extra medical cost to the customer if during package period. Outside of surgery package dates, a hospital fee of 2000 USD minimum plus accommodation will be incurred.

However, Azurite has no legal jurisdiction in any countries which surgery is conducted and any disputes arising out of surgical error is to be relayed to your Customer Representative immediately to ensure efficient handling of the matter. Azurite will do its best to ensure that any disputes occurring out of surgical error are addressed and remediated however Azurite waives the responsibility for any post-operative issues caused by negligence or omission of the customer/ clients.

Terms and Conditions for Superannuation Document Preparation Services

Welcome to Azurite Medical and Wellness. These Terms and Conditions govern your use of our services for the preparation of superannuation documents for medically necessary surgery. By engaging our services, you agree to comply with and be bound by the following terms.

Service Description

Azurite Medical and Wellness provides assistance in the preparation of documents required for superannuation claims, specifically in relation to medical and wellness assessments.

Service Fee

A service fee of $2000 AUD will be charged to clients under the following conditions:

Payment Terms

  • The $2000 AUD non refundable service fee will be charged after 28 days of the issuance of the invoice and supporting specialist letter. Any service fee charged will be applied as a credit to selected surgery package.
Client Obligations

By using our services, you agree to:

  • Provide accurate and complete information as requested for the preparation of superannuation documents.
  • Notify us promptly if there are any changes to your circumstances that may affect your eligibility or the preparation of documents.
Azurite Medical and Wellness Obligations

We agree to:

  • Conduct a thorough assessment of your eligibility for superannuation claims based on the information provided.
  • Provide a supporting specialist letter and invoice if you are eligible and if we assist in the preparation of your documents.
  • Ensure confidentiality and security of all personal and medical information provided by you.
Termination of Services

Azurite Medical and Wellness reserves the right to terminate services if:

  • The client fails to provide necessary information or documentation.
  • The client fails to pay the service fee within the specified timeframe.
Limitation of Liability

Azurite Medical and Wellness is not liable for:

  • Any decisions made by superannuation funds regarding the acceptance or rejection of claims.
  • Any delays or failures in the provision of services due to circumstances beyond our control.
Changes to Terms and Conditions

Azurite Medical and Wellness reserves the right to amend these Terms and Conditions at any time. Clients will be notified of any significant changes.

Governing Law

These Terms and Conditions are governed by and construed in accordance with the laws of NSW.

  1. Non-Proceeding with Selected Surgeons: If a client chooses not to proceed with the surgeons selected by Azurite Medical and Wellness, a service fee of $2000 AUD will be applied.
  2. Eligibility: This fee will not be charged if the client is deemed ineligible for superannuation claims. Official documentation on case will need to be provided.
  3. Supporting Documentation: The fee will only be charged if Azurite Medical and Wellness provides the client with a supporting specialist letter and an invoice.
